What you can add:
- Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
- Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
- Signposting to appropriate care providers (including primary and tertiary care).
- Raising concerns to senior staff for support and management.
- Completing documentation and planning care for patients.
- Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
The role will involve, but not be limited to you performing the above.
What you'll need:
- A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Full UK driving licence and own vehicle.
*Depending on the Trust, you may require variable additional certification and skills.
