Kaleidoscope is a forward-thinking organisation delivering high-quality support to individuals affected by substance misuse, homelessness, and other multiple needs. We are committed to empowering people to achieve meaningful and sustained change in their lives.
Kaleidoscope operates multiple projects across all of Wales and some parts of England, each tailored to meet the unique needs of the communities we serve. The Floating Support Worker will sit within the Kaleidoscope Powys service which provides a range of services for anyone experiencing problems because of substance and/or alcohol use.
This role is funded by Powys County Council Housing Support Grant (HSG) team which provides holistic support to adults (18+) with multiple needs, including but not limited to homelessness, mental health issues, learning disabilities and substance misuse. The HSG aim is to help individuals improve their wellbeing, quality of life, maintain tenancies, and achieve greater independence in the community.
The post holder should enjoy face-to-face contact with individuals they are supporting. They will have excellent communication skills, be able to work as part of a multidisciplinary team, across agency cultures including those within statutory, private and voluntary sector partnerships. The post holder must be flexible and efficient within small locality teams, to ensure services are delivered equitably and with the individual accessing the service at the heart of delivery.
Individual supervision will be provided six-weekly in line with Kaleidoscope policy. Specific training will be available for the development of service provision in line with Continued Professional Development.
Role Purpose:
- To support the Kaleidoscope team to offer flexible, person-centred support as per HSG Support Worker Role using psychologically informed approaches to service users who are struggling to live independent lives and/or who live alone and are vulnerable so they can maintain a safe and secure home.
- To support service users using a psychologically informed peer led approach: using relevant aspects of your own experience to inspire hope in others, where appropriate and being a positive role model for service users overcoming a range of experiences including drug and alcohol use, offending, homelessness and emotional difficulties
- To develop an awareness of local available services and community resources, and establish and maintain positive relationships with these services
- To support service users to develop and maintain meaningful activity, social interaction and community integration, including supporting them access to local services, groups and resources – i.e. in relation to education, training, employment, community living, wellbeing and leisure
Key Duties and Responsibilities:
- Be needs-led and bring equality of access including people with various needs and characteristics.
- Provide holistic, trauma-informed support to individuals with multiple needs, recognising the signs and avoiding and mitigating the impact of Child Adverse Experiences (ACEs) and developing Psychologically Informed Environments (PIEs).
- Coordinate access to a wide range of services including health, housing, benefits, and recovery services.
- Advocate for the Individuals in a way they are accepted into the correct pathways in a timely manner.
- Support individuals to access and maintain appropriate accommodation. Advocate on behalf of individuals where appropriate.
- Provide flexible, person-centred support using psychologically informed approaches.
- Develop and maintain links with local services and community resources.
- Empower service users to take responsibility for their goals and independence.
- Support wellbeing and recognise signs of deterioration or relapse triggers.
- Support audit, service evaluation and research activity as required.
- Develop understanding of drug and alcohol use and emotionally informed environments.
- Conduct weekly, face to face contact with individuals on your caseload.
- Respond promptly to housing support referrals.
- Carry out needs and risk assessments, and co-produce support plans and crisis plans.
- Build and maintain trusting relationships with individuals to promote engagement and empowerment
- Undertake community-based outreach activities to support individuals engagement and wellbeing – this may include accompanying individuals on trips, attending appointments, or participating in therapeutic and recreational activities such as allotment projects, walking groups, or wellbeing sessions.
- Flexible working which includes being office based, conduct home visits to individuals accommodation whilst following lone working procedures, hot desking at local Health building, Libraries and community hubs.
- Record thorough, accurate case notes of each contact and visit you have with individuals. Update and review support plans and crisis plans regularly.
- Report to professionals any change observed in a individuals condition, that may contribute to the safety of the individual or the wider public.
- Take a multi-agency approach to working, building strong relationships with partner agencies including but not limited to Powys County Council Homeless Team, Housing Support Grant Services in Powys, PTHB (Powys Teaching Health Board), Dyfed Powys Police, HMPPS (Her Majesties Prison and Probation Service) and other third sector support agencies.
- Attend multi-agency meetings and case conferences.
- Manage and organise your own calendar to ensure effective delivery of support while maintaining a flexible, responsive approach to individuals needs. This will be under supervision of management.
- Promote harm reduction and recovery.
- Display a calm and patient approach to those individuals in times of crisis.
- To empower people to identify, prioritise and take responsibility for their personal goals, developing independence and dignity and self-respect
- To support people to improve and maintain their wellbeing; to identify personal strengths and social networks, as well as identify signs of deterioration and triggers.
- To attend and contribute to team meetings
- To support the design and implementation of audit, service evaluation and research activity, as required.
- Demonstrate positive attitude and maintain a professional manner at all times.
- Promote healthy lifestyle choices.
- Respond to safeguarding concerns in line with organisational and local authority procedures.
- Contribute to team discussions around risk management and safeguarding, sharing relevant updates and seeking guidance when necessary.
- Attend regular team meetings, 6 weekly supervision, and training as and when required.
- Follow Kaleidoscope policies and procedures, including Health & Safety and Safeguarding.
- Undertake other reasonable duties as required
Pay: £26,227.50 per year
