Job Summary
We are seeking a dedicated and organised Care Coordinator to oversee and coordinate senior care services within our company. The successful candidate will be responsible for developing personalised care plans, supervising staff, rotas and ensuring the delivery of high-quality care. Duties
- Develop, implement, and review personalised care plans tailored to each resident’s needs.
- Coordinate daily care activities, ensuring the highest standards of safety and wellbeing.
- Supervise and support care staff, providing guidance and leadership to promote a positive working environment.
- Liaise with residents, families, healthcare professionals, and external agencies to ensure seamless communication and service delivery.
- Organise staff rotas, monitor attendance, and manage staffing levels to meet operational requirements.
- Oversee medication management and ensure compliance with health and safety regulations.
- Maintain accurate documentation using IT systems, including care records and progress reports.
- Facilitate training sessions for staff on best practices in senior care and compliance standards.
- Ensure adherence to all policies related to safeguarding, confidentiality, and quality assurance.
Qualifications
- Proven supervising or leadership experience within a care industry or senior care setting.
- Strong communication skills, capable of liaising effectively with residents, families, and multidisciplinary teams.
- Driving licence preferred for facilitating resident outings or staff transportation needs.
- Experience in developing and managing care plans tailored to individual needs.
- Good IT skills, including familiarity with electronic health records and scheduling software.
- Knowledge of health & safety regulations relevant to senior care environments.
