First Active 365 Homecare is seeking an experienced and motivated Clinical Lead Nurse to support and oversee our growing portfolio of community-based packages of care.
This is a primarily remote position with travel required for client assessments, staff training, competency sign-offs, and clinical oversight within the community.
The Role
As Clinical Lead Nurse, you will play a vital role in maintaining the highest standards of clinical care across our homecare services. You will work closely with carers, clients, families, and management teams to ensure safe, effective, and person-centred care delivery.
This role is ideal for a nurse who is passionate about leadership, mentoring staff, and improving quality within complex community care environments.
Key Responsibilities
- Conduct assessments for new and existing packages of care
- Deliver clinical training and ongoing support to care staff
- Complete competency assessments and sign-offs
- Provide clinical oversight and governance across care packages
- Review care plans, risk assessments, and clinical documentation
- Support staff performance, development, and best practice
- Liaise with families, healthcare professionals, and multidisciplinary teams
- Ensure compliance with CQC standards and company policies
- Participate in audits, investigations, and service improvement initiatives
Requirements
- Registered Nurse (RGN/RNLD or Childrens Nurse) with active NMC registration
- Previous experience within homecare, complex care, or packages of care
- Experience delivering practical training and competency assessments
- Strong leadership and organisational skills
- Ability to work independently and manage a remote workload
- Full UK driving licence and willingness to travel
- Excellent communication and documentation skills
