Care Coordinator/Field Care Supervisor Dual Role - Bexley

Job Responsibilities:
Care Coordination & Administration:
- Administer and coordinate care services to ensure smooth operations and efficient service delivery.
- Implement and manage care rosters, ensuring staff schedules are complete and updated on the staff management system.
- Allocate suitable Care Workers to client calls, ensuring quality and compliance with company standards.
- Maintain accurate client records and confidentially manage data and information.
- Build rapport with customers and staff to uphold high-quality service delivery.
- Support the Branch Manager with daily operations and overall branch administration.
Field Supervision & Service Quality:
- Complete personalised care plans and risk assessments within 48 hours of receiving care packages before care services commence.
- Arrange and manage care worker changeovers, briefings, and performance evaluations through home visits.
- Ensure Care Workers provide holistic and effective care while adhering to compliance and safety regulations.
- Troubleshoot emergency care call coverage when required.
Compliance & Training:
- Ensure all Care Workers complete mandatory annual training and report non-compliance to management.
- Work within CQC, company policies, and confidentiality guidelines.
- Promote best practices among Care Workers and participate in ongoing training to enhance knowledge and skills.
- Monitor Care Workers' adherence to the company’s Quality Management System.
Financial Responsibilities:
- Report changes in care plans to the Care Coordinator for StaffPlan updates.
- Ensure private client contracts and additional service documentation are promptly submitted to the Branch Manager.
- Oversee the secure collection and processing of payments from clients.
Leadership & Team Support:
- Supervise and organize Care Workers within assigned teams, facilitating shadowing for new employees.
- Assist with recruitment, induction, supervision, and appraisals of Care Workers.
- Lead monthly Care Worker team meetings and maintain excellent internal and external communication.
Client & Community Engagement:
- Liaise with Local Authorities, Next of Kin, and healthcare professionals to support care plans.
- Conduct client reviews and report care plan updates to the Care Coordinator and Branch Manager.
- Actively promote the company’s services through marketing efforts such as leaflet distribution and networking within the healthcare community.
Quality & Risk Management:
- Monitor staff performance in collaboration with the Quality Assurance Officer and Care Manager.
- Identify and report concerns related to abuse, neglect, or safety risks immediately to the Branch and Operations Managers.
On-Call & Emergency Coverage:
- Provide on-call support as required to ensure service continuity.
- Assist in troubleshooting and covering care calls during emergencies.
This dual role integrates care coordination and field supervision, ensuring an efficient, high-quality care service while maintaining regulatory compliance and team leadership.
Requirements:-
- Experience in Care is a must.
- Full UK Driving Licence and vehicle is must.
- Excellent communication skills; listening, verbal, and written.
- Great team player.
- Excellent customer service skills.
- Flexible and adaptable.
- Effective organizational and multi-tasking skills.
- Awareness of key business principles.
- Excellent IT skills.
- Ability to work under pressure.
