Continuing Healthcare Specialist Practitioner / Nurse (Band 6)

Working from office and out in the surrounding community
Pay Rates: £20.88 per hour
Shift Patterns: 37.5 hours a week Monday – Friday 09:00 – 17:00
Part time hours may be considered preferable +24 hours per week
We are looking for a registered professional who is experienced in working with people who have complex health needs
The post holder will be responsible for assessing and determining eligibility for people with complex health needs who are eligible for NHS funding for long term care and treatments under The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, The Mental Health Act. The service also commissions packages of care for clients requiring Transforming Care and Neurological Rehabilitation pathways.
The post holder will be responsible to clinically plan and commission the care required, ensuring that the care is safe, effective and good quality. Communication and relationships with individuals and independent providers is a key part of this role as well as abilities to manage risks, problem solve influence and negotiate to ensure appropriate clinical commissioning case management of complex individuals.
The post holder will chair multi-disciplinary meetings in order to make eligibility recommendations for NHS Continuing Healthcare and NHS Funded Nursing Care. Complete reviews of commissioned packages ensuring that people are receiving high quality safe and effective care as well as ensuring they still have eligibility for the relevant funding stream. You will also provide expert advice to other professionals commissioned to provide care in relation to NHS continuing Healthcare and NHS Funded Nursing Care packages
You’ll need to have the following qualification/experience:
- Registered Nurse - General/Adult/Mental health/Learning Disability
- Minimum of 6 months experience as a Nurse Assessor within the last 3 years
- NMC Registered
- Educated to degree level or appropriate equivalent
- Experience of carrying out complex nursing/healthcare assessments for clients with health and social care needs.
- Experience of care planning for clients with complex health and social care needs.
- Experience of working in partnership between health and social care and wider system partners.
- Fully conversant with the assessment process and need to assess, plan, implement, and evaluate the needs of clients
- Ability to travel between locations across Oldham & Greater Manchester as needed (Car user - access to a vehicle and ability to travel on a daily basis to assessments as required.
What you’ll be responsible for
- The post holder will act as CHC commissioning nurse for patients whose care is either jointly or solely funded by NHS GM and the care is provided by the independent sector
- Identify and case manage clients who may be eligible for NHS funding, chair and coordinate eligibility assessments and reviews
- Provide expert advice to other professionals commissioned to provide care in relation to NHS Individualised Commissioning
- Complete the restitution process for all claims which includes presentation of findings to the NHS GM multi-agency funding panel and the NHS England Independent review panel
- Contribute to the quality monitoring of care home providers
- Support with the duty function of the service
- Understand and communicate complex information, both patient and non-patient related
- Communicate complex and often sensitive and distressing information on a regular basis
- Manage complex, sensitive and sometimes contentious situations
- Work autonomously, prioritising and managing own workload
- · Undertake assessments in accordance with the National Framework using clinical knowledge and expertise to assess patient’s health care needs and determine care requirements
- Provide expert in relation to NHS Continuing Healthcare advice, support to a defined group of patients and liaise as appropriate with lead health professionals
- Have the ability to positively challenge and influence health and social care professionals to ensure that plans for patient care are appropriate and that the patient will be cared for on the most appropriate pathway where possible utilising the least restrictive options
- Identify appropriate care provision and request provider assessments
- Ensure that patients are given appropriate information in relation to Personal Health Budgets in accordance with NHS GM Policy
- Undertake patient reviews in a timely manner ensuring that this includes a review of patient care, care records and care provision to ensure commissioned care is appropriate, of acceptable quality and meets the patients identified care needs
- Act appropriately when there are requirements to re-consider care provision. Make recommendations of changes to provision and interim solutions if required
- Have involvement in safeguarding investigations for NHS funded clients. Provide support to patient’s and consult with other practitioners and associated staff to ensure high quality care
