Mitcham & Tooting Health is seeking an experienced and patient-focused Clinical Pharmacist to join our growing multidisciplinary team. This is an excellent opportunity for a motivated pharmacist looking for a flexible role within a supportive and forward-thinking primary care environment.
We are looking for a Clinical Pharmacist who is confident managing patient consultations, supporting medicines optimisation, and working collaboratively with clinicians to deliver high-quality care to our population of over 8,000 patients.
Main Responsibilities
Key duties will include:
- Conduct face-to-face and telephone consultations, including management of minor illnesses and online triage requests.
- Promote evidence-based and cost-effective prescribing across the practice.
- Reconcile medications following hospital discharge and specialist clinic attendance.
- Review and manage medications for newly registered patients.
- Support clinicians in managing patients requiring substance misuse and dependency care.
- Review and implement medicines safety alerts and associated actions.
- Lead on repeat prescribing processes, ensuring safe and efficient systems are maintained.
- Reauthorise repeat prescriptions within scope of practice and identify patients suitable for repeat prescribing programmes.
- Implement medication safety protocols for high-risk and vulnerable patients, including those with complex medication regimens.
- Conduct targeted medication reviews for patients with polypharmacy, frailty, and long-term conditions.
- Provide expert advice on medication monitoring and medicines optimisation.
- Support achievement of Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) targets.
- Signpost patients appropriately to other healthcare professionals and services.
- Work collaboratively with the multidisciplinary team to deliver safe, effective, and patient-centred care.
- Participate in audits, training, prescribing initiatives, and quality improvement projects.
The Ideal Candidate
To be considered for this role, you will have:
Essential
- Current registration with the General Pharmaceutical Council (GPhC).
- Minimum of 2 years' experience managing GP triage and/or patient consultations within a primary care setting.
- Strong commitment to delivering high-quality, patient-centred care.
- Evidence of ongoing professional and personal development.
- Excellent communication, clinical assessment, and decision-making skills.
Desirable
- Independent Prescriber qualification (preferred but not essential).
- Experience working within General Practice or Primary Care Networks.
- Working knowledge of EMIS Web and Docman.
- Experience supporting long-term condition management and medicines optimisation programmes.
