This is a full-time position, working five days per week (Monday to Friday). The role operates on a rotational shift pattern, covering 08:00–16:00 and 10:00–18:00
You will be employed by the Flow Hub at Broomfield Hospital and will work as part of a team of Patient Flow Co‑Ordinator's, providing operational support to a wide range of wards across the hospital. The role plays a key part in supporting patient flow and ensuring the safe and effective movement of patients within the organisation
To ensure all relevant social service referrals are made in a timely manner, response times are monitored, and delays acted upon and work with the ward team to ensure a smooth patient flow with regards to discharge.
To be a point of contact for the ward with regards to discharge and to ensure they carry out the specific responsibilities as outline within this Job Description.

The Patient Flow Co-Ordinator role is a varied role with a primary focus on patient flow and patient discharge. You will be point of contact on the wards for information and advise to staff, patients, and relatives/visitors.
You will provide advice and patient management relating to complex discharge scenarios and work in a collaborative manner with the multidisciplinary team to facilitate appropriate and timely discharges by providing continuity in discharge planning and effective dissemination of information.
You will work collaboratively with the Multi-Disciplinary Team (MDT) to facilitate safe and timely discharges by providing continuity in discharge planning and effective dissemination of information.
Day-to-day responsibilities will include but not limited to; attending MDT Board Rounds and collating detailed information about each patient, escalating diagnostics and speciality reviews, patient transport bookings, communicating with patients and their families, liaising with pharmacy, ensuring there is a clear plan from all in the MDT and provide effective written documentation about discharge progress.
You will be computer literate; have excellent communication skills both written and verbal.
