About the Role
Moonrise 24 Hr is seeking a dedicated and experienced Team Leader to join our growing domiciliary care team. This is an exciting opportunity for a motivated individual who is passionate about delivering outstanding person-centred care while supporting and leading care staff in the community.
As a Team Leader, you will play a key role in ensuring the safe, effective, and high-quality delivery of care services. You will provide supervision and support to care staff, conduct service user visits, monitor care standards, assist with rota management, and contribute to safeguarding and quality assurance processes.
Key Responsibilities
- Lead, motivate, and support care staff to deliver high-quality, person-centred care.
- Conduct regular spot checks, observations, and quality monitoring visits within service users' homes.
- Support the coordination of care services and assist with staff scheduling and rota management.
- Carry out service user assessments, reviews, and care plan updates as required.
- Monitor care delivery to ensure compliance with company policies, regulatory requirements, and care standards.
- Identify, assess, and manage risks relating to service users and care delivery.
- Act promptly on safeguarding concerns and contribute to safeguarding investigations and assessments.
- Provide supervision, mentoring, and guidance to care staff to support performance and professional development.
- Maintain effective communication with service users, families, healthcare professionals, and external agencies.
- Support the resolution of service delivery issues, staffing shortages, and emergency situations.
- Participate in the on-call rota and provide out-of-hours support when required.
- Ensure accurate and timely completion of documentation, reports, and care records.
- Promote dignity, independence, wellbeing, and positive outcomes for all service users.
Essential Requirements
- Previous experience in a Team Leader, Senior Care Worker, Care Coordinator, or similar supervisory role within domiciliary care.
- Strong understanding of person-centred care and safeguarding practices.
- Knowledge of CQC standards and health and social care regulations.
- Excellent communication and leadership skills.
- Ability to effectively supervise and support care staff.
- Strong organisational and problem-solving abilities.
- Competent in maintaining accurate records and using care management systems.
- Full UK driving licence and access to a vehicle.
- Flexibility to participate in the on-call rota.
Desirable Qualifications
- NVQ/QCF Level 3 in Health and Social Care (minimum).
- NVQ/QCF Level 5 in Leadership for Health and Social Care or willingness to work towards it.
- Experience conducting care assessments and reviews.
- Experience supporting recruitment, training, and staff development activities.
