We are looking for a worker who has an excellent values base, and has previous experience of working professionally in a caring role.
You would carry a caseload of clients with complex mental health needs in the community who you work with on a 1:1 basis, this may include but is not limited to:
- Build Relationships
- Assessment and Goal Planning/ Personal Pathways
- Person Centred Support Planning
- Risk Management
- Outcome Focused support
- Advice and Guidance
- Referral and Signposting to other avenues of support
- 1:1 Community work with Flexible caseload.
You will be supported by Senior Staff to carry out this role with regular supervision sessions. Working hours are Monday – Friday 9am – 5pm and excludes bank holidays. 22.5 hours per week only.
