Overview
We are seeking a dedicated and compassionate Social Prescriber (PCN) to join our multidisciplinary healthcare team. This role offers an exciting opportunity to support individuals in improving their health and wellbeing through personalised, holistic approaches. The successful candidate will work closely with patients, healthcare professionals, and community organisations to facilitate access to social, emotional, and practical support. Duties
- Conduct comprehensive assessments of patients’ social, emotional, and practical needs.
- Develop personalised support plans in collaboration with patients, considering their unique circumstances.
- Provide information and signposting to relevant community services, activities, and resources.
- Facilitate group sessions or workshops on topics such as nutrition, mental health, or lifestyle changes.
- Build strong relationships with patients to encourage engagement and adherence to support plans.
- Collaborate with healthcare professionals and community organisations to coordinate care pathways.
- Maintain accurate records of patient interactions and support plans in accordance with organisational policies.
- Promote health education through public speaking engagements and community outreach initiatives.
Qualifications
- Proven experience or background in nursing, public health, teaching, or related fields.
- Knowledge of medical terminology and healthcare systems is advantageous.
- Strong communication skills with the ability to engage effectively through public speaking and one-to-one interactions.
- Understanding of nutrition and its impact on overall health is desirable.
- Organisational skills to manage caseloads efficiently whilst maintaining attention to detail.
- Compassionate approach with a genuine interest in supporting community wellbeing.
- Ability to work independently as well as part of a multidisciplinary team.
