PRACTICE NURSE – Between 30 hours and 37.5 hours
Monday to Friday - working pattern to be agreed but will include some 8:30am starts and 6:30pm finishes.
We are looking for an enthusiastic Practice Nurse to join their high achieving team of:
6 GPs
2 Advanced Practitioners
2 Physician Associates
4 x Practice Nurses
1 x HCA
1 x GPA
Full admin support
Outstanding CQC practice – November 2018
Practice list size 13,500
Clinical system – EMIS Web.
Salary dependent upon experience
NHS Pension Scheme.

This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the patient population.
Working autonomously, the Practice Nurse will be responsible for a number of clinical areas including:
Chronic Disease Management
Family Planning,
Cervical Cytology,
Cardiovascular Health Screening,
Public health and screening programmes
Infection prevention and control
Health promotion and prevention
Vaccinations
General Treatment Room duties
Remote telephone and face to face consultations
Digital correspondence with patients using AccuRX
Furthermore, they embed population health management approaches within general practice, targeting health inequalities and improving access.
