About the Role
We are recruiting on behalf of a reputable Housing Association for a Support Worker to join their team on a permanent basis.
This is a community-based role, supporting individuals at risk of homelessness across Wakefield. You will work both in service users’ homes and deliver 1:1 sessions from the head office, helping individuals sustain tenancies and move towards independence.
Key Responsibilities
- Supporting individuals at risk of homelessness, including those with complex needs, substance misuse, or alcohol-related issues
- Creating, implementing, and reviewing personalised support plans
- Encouraging and monitoring engagement and adherence to support plans
- Conducting home visits and risk assessments
- Delivering 1:1 support sessions from the Wakefield office
- Working in partnership with external agencies and services
- Maintaining accurate case notes and records
Requirements
- Experience working with vulnerable individuals or in a similar support setting
- Understanding of homelessness, substance misuse, and complex needs
- Full UK driving licence and access to a vehicle (essential)
- Enhanced Adult DBS issued within the last 3 years
- Strong communication, organisation, and safeguarding awareness
