We are looking for an experienced and dynamic clinician to join NHS Hampshire and Isle of Wight ICB as a Clinical Lead for Primary Care and Community.
This is a high-impact, system-facing leadership role where you will help shape, design and transform primary and community care services across the system. You will play a critical role in improving access, outcomes, patient experience and health equity for our population, working closely with partners across the Integrated Care System (ICS).
You will provide both:
Place-based leadership for primary care
System-wide clinical leadership across key transformation priorities
This role offers a unique opportunity to influence strategic direction, support neighbourhood models of care, and drive innovation in how services are delivered.
A key element of this role will be providing credible clinical leadership within Portsmouth locality. The post holder will be expected to work within the area that they are appointed to support, bringing a strong understanding of local primary care, community services, population needs and clinical relationships.
Provide clinical leadership for primary care at Place level and contribute to system-wide priorities, improving outcomes, access and patient experience.
Lead and support the design and delivery of transformation programmes, including service development, improvement plans and new care models.
Support the development of modern, neighbourhood-based models of general practice, aligned with system strategy.
Work collaboratively with PCNs, practices, providers, local authorities and VCSE partners to drive integrated care and service change.
Working closely with clinical leads from other localities within NHS HIOW and within the directorate and wider teams.
Provide clinical oversight and assurance of primary care services and contracts, ensuring delivery against national and local performance standards.
Contribute to tackling health inequalities and improving population health, including support to equity programmes and prevention initiatives.
Ensure robust clinical governance, risk management and programme reporting, supporting quality and continuous improvement.
Act as a visible clinical leader, maintaining strong engagement with clinicians and stakeholders, and supporting co-production with patients and communities.

While the role is particularly suited to an experienced practicing GP, applications are welcomed from clinicians who can demonstrate the local credibility, relationships and insight required to lead effectively across practices, PCNs and wider system partners.
Provide clinical leadership for primary care within a defined Place, contributing to the development of safe, effective and integrated services.
Offer system-level clinical leadership across assigned local care priorities,
supporting delivery of ICS strategic objectives and population health
ambitions.
Support the development and evolution of modern general practice, ensuring alignment with neighbourhood-based models of care.
Promote evidence-based, person-centred approaches that drive improvement in access, outcomes and patient experience.
Support the system’s response to health inequalities, including input to
Core20PLUS5 delivery and local equity initiatives.
Oversee the clinical management and assurance of Primary Care Contracts and services (GMS/APMS/PMS)
Working closely with Practices and Primary Care Networks, the GP
Federations, NHS England and other primary care contractors, the post holder will establish the implementation of the Primary Care Strategy, ensuring the effective implementation of projects with maximum buy-in/engagement from practices and PCNs.
To maintain performance monitor guaranteeing high quality care including the delivery of national plan targets, local targets and key national standards including those within National Service Frameworks and the recommendations of NICE guidance relevant to primary care.
