Our domiciliary care services provide care and support to individuals or families in their own home. The service is required due to infirmity, illness, incapacity or social problems that prevent our clients from undertaking their various self-care duties. Our aim is to promote and prolong our client’s independence in their own home.
Predominantly, our client base is elderly, though illness effects all ages, therefore you should be able to care for clients from the age of 18 years old.
Our social care work services provide personal care and support. Heritage Healthcare also employ support staff responsible for assisting with domestic duties in the client’s home.
Care work is a career. Our care workers receive practical training to complete their duties, as well as studying for Certificates and Diploma’s in care, so as to promote their career development.
Duties
- To travel to the residence of clients who are having difficulty coping for a variety of reasons;
- To complete all services for that client, as specified in their individual Care Plan
- Supporting the registered manager and Care coordinator to ensure company policies & CQC regulations are being adhered to.
- To encourage and motivate clients to maintain their independence;
- To carry out personal and caring tasks as depicted in the Care Plan that would normally be carried out by the client or family.
- To assist in maintaining a good level of personal hygiene for the client who may have difficulties in this area.
- To work and communicate with family, relatives, local authority social workers, district nurses and other members of the care team.
- To contact emergency care services in cases of emergency.
- Responsibility for ensuring that the care services provided by the staff comply with the policies and regulations.
- Responsibility of ensuring that all client documents reflect the care needs of the clients.
- To document and report to the Care Coordinator / Registered Manager, on the welfare of the client.
- To attend all study courses that are organised.
- To ensure care staff complete all services for that client, as specified in their individual Care Plan;
- Work in the community for at least 20 hours a week on an allocated rota and carry Oncall responsibility as per company policy and oncall duty is inclued in your annual salary.
- To work and communicate with family, relatives, local authority social workers, district nurses and other members of the care team;
- To inform care staff of changes to care plan and interventions required
- To ensure all care visits are attended by an appropriately trained and experienced carer
- To ensure carers are given information about the service user, their difficulties and the roles they are to perform prior to their first attendance.
- To ensure all care visits have the agreed number of carers at all times and visits take place at the pre-arranged times
- In cases of staff shortages and in emergencies act in the role as Care Worker ensuring per-arranged care visits take place.
Further Duties
- To act in accordance with all Heritage Healthcare operational policies and procedures.
- To work within the hours agreed on the rota system, which includes Bank Holidays, weekends and evenings. Heritage Healthcare attempt to provide working times suitable for employees, though this is not always possible.
- To be contactable via a mobile phone if necessary, information needs to be communicated to yourself about a client.
- Having the on-call phone on a rota basis
- Employees should be accountable to the care manager or their delegated deputies.
- Assisting the care coordinator / Registered Manager in setting up new care plans and continuous monitoring/updates to client files.
- Assisting the manager/care coordinator to ensure all carers are in compliance with the company policies.
- Carry out staff competency checks including medication and manual handling.
- Carry out staff observation and spot checks.
- To investigate concerns relating to the service.
- To participate in service quality meetings/ Client reviews.
- To work together to constantly improve services.
Conditions
- Heritage Healthcare require a DBS prior to commencement of work, which the company will apply for;
- Offers of employment are subject to the Company receiving three written references, one of which being from your last employer;
- All posts are subject to a 3 month probationary period.
