Job Title
Care Navigator Apprentice Hours: 24 hours per week (Monday, Tuesday,
Wednesday) Contract: Fixed-term apprenticeship (typically 12-18 months, dependent
on qualification level) Salary: Apprenticeship wage (in line with national apprenticeship
rates), plus dedicated training time Location: Addington Road Surgery

The role
As a Care Navigator Apprentice, you’ll be one of the first points of contact for our
patients, whether face-to-face, by phone, or online. Alongside your day-to-day duties,
you’ll work towards a recognised qualification (e.g. Level 3 Customer Service
Practitioner), with structured support, mentoring, and protected time for coursework
and training.
Key responsibilities
Greet patients and visitors warmly and professionally
Answer telephone calls and manage appointment bookings, amendments, and
cancellations
Learn to use clinical systems (e.g. EMIS) to update patient records
accurately Signpost patients to the most appropriate service (GP, nurse, pharmacy, social
prescribing, or community support)
Process prescription requests and respond to general enquiries
Attend training days and complete apprenticeship coursework as required

No previous experience needed, just enthusiasm, reliability, and a willingness to
learn
Friendly, caring manner and good communication skills
Comfortable using computers and confident learning new systems
Commitment to completing the apprenticeship programme
