An exciting opportunity has arisen at Bodmin Community Hospital for a Progress Co-Ordinator to join the Clinical Assessment Treatment Unit (CATU) Team at Bodmin Community Hospital. You will play an important role in supporting the clinical staff in facilitating safe and timely discharge from hospital for patients.
You will help provide a seamless service for patients which will include cross boundary and collaborative working with the multi-disciplinary team along with colleagues in Adult Social Care and other outside agencies in the community setting.
This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.

This post to include occasional weekends and bank holidays on a rota basis. Shift split over the week to be discussed at interview and agreed with successful applicant.
To assist with the efficient day to day running of discharges, providing administrative and clerical support to the team, providing a 7-day service. This post will be required to work flexible hours to meet the requirements of the post depending on service provision.
To monitor patients, progress and report back progress / inform the team or referrer of any problems regarding timing of interventions, including equipment ordering /delivery and organisations of on- going care.
To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards the chosen home environment.
