Smeaton Healthcare is growing, and we are excited to offer a fantastic opportunity for an efficient, organised and experienced Care Coordinator to join our highly respected team in Plymouth. This role is central to the smooth running of our service and requires someone who is proactive, compassionate, and confident in managing a fast-paced environment.
The role of the Care Coordinator is to working within our short visits team.
Key Responsibilities
As a Care Coordinator, you will play a vital role in ensuring our customers receive safe, consistent, high-quality support. Your duties will include:
- Supporting Team Leaders, field care supervisors, Homecare Assistants, providing ongoing support, guidance and oversight.
- Organising and coordinating care services, ensuring all visits are delivered effectively and in line with customer needs.
- Planning and allocating care runs/shifts to the most appropriate staff members.
- Managing bookings and responding to customers requests in a timely, professional manner.
- Liaising with external professionals (e.g. social workers, GPs, community nurses) to support the delivery of person-centred, holistic care packages.
- Participating in the out-of-hours on-call rota as required.
- Safeguarding responsibilities, including recognising, reporting and escalating concerns — experience in this area is essential.
- Covering frontline care in exceptional circumstances, such as sickness or emergencies.
- Supporting management with day-to-day operational tasks.
- Completing staff supervisions and appraisals, ensuring high standards of practice.
- Recruitment support, including interviewing and onboarding new staff.
- General administration and maintaining an organised, compliant office environment.
Requirements
We are looking for someone who can demonstrate:
- Full UK driving licence & access to a vehicle
- Level 3 Health & Social Care (or willing to work towards)
- Previous experience in a similar care coordination or senior care role
- Strong safeguarding knowledge and experience
- Excellent IT and administrative skills
- Flexibility to support service needs
- Willingness to travel, if required
- Commitment to participating in the on-call rota
