We’re looking for an experienced Practice Nurse's to support the Targeted Demand Management (TDM) service, delivering community‑based care from a mobile unit across Blackpool.
The hours will be Monday – Thursday, 08:30 – 17:00 – any availability will be considered.

Key focus:
- Long‑term condition reviews, particularly COPD / respiratory and CVD
- Spirometry and FeNO
- Holistic assessments and health prevention interventions
- Working autonomously in a non‑traditional community setting
- Accurate clinical documentation on EMIS (essential)
Requirements:
- NMC‑registered Practice Nurse
- Strong LTC and respiratory experience
- Confident using EMIS Web
