As part of our skill mix team, you will contribute to the delivery of the national Healthy Child programme across Hounslow, working closely with partners, such as GPs, Family Hubs & children’s centres and social care providing seamless care to the population of Hounslow.
This role combines clinical practice with leadership, supervision, and service development responsibilities. The postholder will ensure the provision of safe, effective, and high-quality health visiting services that promote the health and wellbeing of children and families.
Your Role Will Include:
Caseload Management:
Oversee a defined, complex caseload of families with additional needs
Provide expert assessment and early intervention in line with the Healthy Child Programme
Champion equitable access to services for vulnerable families
MECSH Programme Delivery:
Deliver the MECSH programme with fidelity, supporting sustained and structured home visiting
Facilitate improved outcomes for families through evidence-based parenting interventions
Contribute to ongoing programme evaluation and outcome monitoring
Supervision and Professional Leadership:
Work effectively within a skill mix team
Promote reflective practice and continuous professional development
Contribute to local service development, audits, and quality improvement initiatives
The post holder will carry overall responsibility for individuals, families and communities in line with contractual agreements and within their sphere of responsibility. Working in collaboration with members of the primary health care team, allied health professionals and other key members of statutory and voluntary agencies are key components of this role.
