The Memory Assessment Service in Cornwall Partnership NHS Foundation provides assessment and treatment to people living with dementia or suspected dementia.
We are looking for a passionate, highly motivated individual that is an effective communicator, and with considerable experience in the role they are applying for, or experience in assessment of people with dementia.
This post will work alongside the local complex care multi-disciplinary community teams, these form an essential part of the pathway from early diagnosis.
The post is supported by the Dementia and Older People's Mental Health team, the Primary Care Practitioners, Occupational Therapist and the Dementia liaison nurse providing care for people across the pathway.
This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.

To provide specialist clinical knowledge and specialist skills in assessment, for people referred
with a suspected dementia
- To identify, assess and support the diagnosis of dementia process
- To support and monitor treatments prescribed
- To provide post diagnosis counselling and support to people and their carers following the
assessment and diagnosis
- To work in partnership with Consultant and general medical colleagues, ensuring physical and
mental health needs are considered holistically
- To ensure that the person’s needs are meet throughout the period of assessment, by ensuring
liaison with and signposting to other services if required
- To use a person centred approach that promotes independence and well-being minimizing the
need for medical intervention.
- Assess, plan, implement and evaluate individual care plans in partnership with service users,
carers, and other members of the multidisciplinary team where appropriate
- Produce individual treatment plans and work within protocols for the use of Cognitive Enhancer
medication
