As a Senior Social Worker within our Short Term Support Service, you will play a key role in facilitating safe, timely, and person-centred discharges from hospital settings.
Working closely with health professionals, families, and wider partners, you will ensure individuals transition from hospital to the most appropriate care setting, promoting recovery, independence, and a positive discharge experience.
In this role, you will manage a complex caseload, applying your professional expertise to support effective decision-making and high-quality outcomes. You will also provide guidance and support to Social Workers, contributing to their development and ensuring consistent best practice across the team.
