Solsken Ltd is currently recruiting an experienced Personal Care Assistant to join a dedicated care team in High Green - Sheffield, supporting an adult male in his own home following a stroke. This is a rewarding, person-centred role focused on promoting independence, dignity, confidence and quality of life.
This is more than a care role. You will be part of a small, consistent team supporting the individual with his rehabilitation journey, daily routines, personal care and meaningful activities. The right person will be patient, encouraging and proactive, with the ability to motivate and support the individual to engage with therapy-led routines, physio exercises and day-to-day goals.

About the Role
You will be supporting an adult male with stroke rehabilitation and daily living within his own home. The role includes personal care, support with routine, meal preparation where required, companionship, household tasks and encouragement to participate in activities that support his well-being and rehabilitation.
A key part of this role will be supporting the individual to remain motivated with his therapy-led programme, including routine exercises and physiotherapy guidance, in line with care plans, risk assessments and professional advice.
Previous experience supporting someone following a stroke, brain injury or with rehabilitation needs would be beneficial, but what matters most is your approach. We are looking for someone who is calm, positive, dependable and able to encourage progress in a respectful and person-centred way.
Shifts Available
- Monday: 08:00–16:00
- Tuesday: 07:00–17:00
- Saturday: 09:00–12:00
- Sunday: 09:00–12:00
Additional hours may also be available to cover sickness and annual leave.
What You’ll Be Doing
In this role, you will support an adult male in his own home following a stroke, helping him to maintain routine, build confidence and continue working towards his rehabilitation goals. You will provide support in a safe, respectful and person-centred way, always following the individual’s care plan, risk assessments, therapy guidance and identified outcomes.
Your responsibilities will include:
- Supporting the individual with personal care in a respectful, dignified and person-centred way, promoting privacy, choice and independence at all times.
- Assisting with daily living tasks, household routines and meaningful activities in line with the individual’s preferences, abilities and agreed outcomes.
- Supporting the individual to follow therapy-led routines, including physiotherapy exercises, stretches and rehabilitation activities as directed within the care plan and professional guidance.
- Providing gentle encouragement, reassurance and motivation to help the individual remain engaged with his rehabilitation journey.
- Promoting independence wherever possible, while recognising when support, reassurance or intervention is needed to maintain safety and well-being.
- Following all care plans, risk assessments, moving and handling guidance, therapy plans and company policies to ensure care is delivered safely and consistently.
- Supporting with safe mobility, transfers and positioning where required, in line with training, competency sign-off and risk assessments.
- Observing for any changes in the individual’s health, mood, communication, mobility, skin integrity, eating and drinking, or general presentation, and reporting concerns promptly.
- Helping to keep the individual safe by following escalation procedures, safeguarding guidance and agreed clinical protocols.
- Maintaining accurate, factual and up-to-date care records in line with company policy, CQC requirements and relevant health and social care legislation.
- Communicating effectively with the individual, family members, healthcare professionals, managers and colleagues to support safe, joined-up and person-centred care.
- Working in line with CQC regulations, the Health and Social Care Act, safeguarding responsibilities, company policies and all relevant legislation.
- Providing companionship, consistency and encouragement while respecting the individual’s home, choices, routines and dignity.

- Confident supporting an adult male with personal care in a respectful and person-centred way.
- Comfortable supporting rehabilitation routines, including physio exercises and activities set out within the individual’s care plan.
- Able to follow care plans, risk assessments, moving and handling guidance, therapy guidance and company policies at all times.
- Committed to working safely and in line with CQC regulations, safeguarding responsibilities and relevant health and social care legislation.
- Observant and able to recognise and report changes in health, mood, mobility, communication or general presentation.
- Confident maintaining professional boundaries while building a positive and supportive working relationship.
- Reliable, consistent and professional, understanding the importance of routine and continuity for the individual.
- Confident communicating with the individual, family members, healthcare professionals, managers and colleagues.
- Able to work independently as well as part of a small, dedicated care team.
- Eligible to work in the UK.
Essential Requirements
- Strong verbal and written communication skills.
- Ability to work both independently and as part of a team.
- Two valid references.
- Two forms of identification.
- Proof of eligibility to work in the UK.
- Willingness to work towards a Level 2/3 Health and Social Care Diploma.
Preferred, But Not Essential
- Full UK driving licence and access to a vehicle.
- Previous care experience.
- Experience supporting adults with stroke rehabilitation needs.
- Experience supporting individuals with brain injury, mobility needs or complex care needs.
- NVQ Level 2/3 or equivalent in Health and Social Care.
- DBS certificate on the Update Service.
