Surrey and Borders Partnership NHS Foundation Trust is seeking bank staff to provide physical and psychological care to service users in line with their agreed clinical care plans, as well as offering support to carers.

Observe, report, and record changes in clients’ physical health, mental health, and behaviour.
Deliver care and support in line with agreed care plans and Multi-Disciplinary Team (MDT) guidance.
Work in accordance with the principles of the Mental Health Act (MHA).
Communicate effectively with clients, relatives, colleagues, and external professionals by telephone and face-to-face contact
Support clients with daily living activities, including personal hygiene, nutrition, hydration, clothing, laundry, sleep, mobility, and maintaining a safe environment.
Provide emotional support, therapeutic engagement, and social activities while maintaining professional boundaries.
Assist in the assessment, planning, implementation, and evaluation of individual care programmes.
Develop and maintain positive professional relationships with colleagues, agencies, and healthcare professionals to support recovery-focused care.
Carry out escort duties for clients under the direction of the nurse in charge.
Complete observations and monitoring in line with individual care plans and Trust policies.
Support and co-facilitate therapeutic and social group activities where required.
Undertake and record prescribed physical observations and promptly report concerns to the nurse in charge.
Complete the Trust’s mandatory Care Certificate Induction Programme where applicable.

Monitor and accurately document any changes in clients’ mental state, physical wellbeing, mood, behaviour, and overall presentation, escalating concerns appropriately.
Provide person-centred care in accordance with agreed care plans, MDT decisions, and organisational policies.
Maintain awareness of the Mental Health Act (MHA) and ensure care is delivered safely, lawfully, and respectfully.
Communicate clearly and professionally through verbal, written, telephone, and face-to-face interactions.
Assist clients with activities of daily living, including: Personal hygiene and grooming, Nutrition and fluid intake, Dressing and laundry, Maintaining a clean and safe personal environment, Sleep and rest routines, Mobility and physical activity, Toileting and elimination needs
Respect and promote each client’s individuality, dignity, privacy, cultural background, values, sexuality, and spiritual beliefs.
Encourage participation in meaningful social, therapeutic, and recreational activities to promote wellbeing and recovery.
Build professional and therapeutic relationships with clients while maintaining clear professional boundaries at all times.
Contribute to the assessment, planning, implementation, and evaluation of care and recovery plans.
Work collaboratively with nursing staff, MDT members, external agencies, carers, and other professionals to ensure continuity and quality of care.
Carry out escort duties for clients attending appointments, activities, or leave arrangements as directed by the nurse in charge.
Conduct observations in accordance with individual care plans, client risk levels, and HPT Trust policies.
Participate in and, where appropriate, co-facilitate therapeutic groups and structured engagement activities.
Undertake and accurately record physical observations, including: Blood pressure, Temperature, Pulse, Respirations, Blood glucose monitoring, Urinalysis, Weight and height measurements
Promptly report all physical observation results, concerns, or abnormalities to the nurse in charge.
Complete all mandatory training requirements, including the Care Certificate Induction Programme where applicable.
