Working with a community nursing team, you’ll deliver high-quality care that helps people stay safe and well at home. You’ll play a key role in supporting people with long-term conditions, coordinating care and reducing avoidable hospital admissions.
You’ll work closely with colleagues across health and care services to plan and deliver personalised care. This role makes a real difference by supporting patients and carers to manage their health and maintain independence.
What you’ll do:
Assess patient needs and plan, deliver and review care in the community
Coordinate care across services, acting as a key contact for patients and carers
Carry out clinical procedures such as wound care and long-term condition management
Maintain clear and accurate Electronic Patient Records (EPR)
Support team planning and help prioritise workloads
What we’re looking for:
Registered nurse with relevant experience in community or long-term condition care
Knowledge of care planning, assessment and evidence-based practice
Confidence working both independently and within a multidisciplinary team
Strong communication skills to support patients, carers and colleagues
Commitment to learning, development and high standards of patient care
