Continuing Healthcare Specialist Mental Health Nurse (Band 7)

Pay Rates: £25.71 per hour
Shift Patterns: 37.5 hours a week Monday – Friday 09:00 – 17:00
Part time hours may be considered preferable +24 hours per week
The aim of this post is to provide complex case management for the mental health and learning disability caseload of the Continuing Healthcare team.
As a registered mental health nurse, the post holder will provide relevant mental health expertise to the continuing health care team, as well as specifically manage mental health and learning disability patient caseload. Case management will ensure that the commissioned package of care is appropriate and meets the needs of the patient. In addition, as case manager, you will ensure that the quality of care provided by commissioned providers meets the required standards
You’ll need to have the following qualification/experience:
- Registered Nurse - Mental health
- Minimum of 6 months experience as Continuing Healthcare Specialist within the last 3 years
- NMC Registered
- Educated to degree level or appropriate equivalent
- Experience of carrying out complex nursing/healthcare assessments for clients with health and social care needs.
- Experience of care planning for clients with complex health and social care needs.
- Experience of working in partnership between health and social care and wider system partners.
- Fully conversant with the assessment process and need to assess, plan, implement, and evaluate the needs of clients
- Ability to travel between locations across Oldham & Greater Manchester as needed (Car user - access to a vehicle and ability to travel on a daily basis to assessments as required.
What you’ll be responsible for
- This role requires a subject matter expert in Mental Health and Learning Disabilities. This includes the clinical knowledge to challenge care providers (including all members of a multidisciplinary team) regarding how the needs of an individual should be met.
- The post is responsible for providing professional expertise for mental health to the outcome of commissioning business processes for the Department or Directorate.
- The post-holder will solely be responsible for the commissioning of complex mental health and learning disability care packages for patients for whom Stockport holds a commissioning responsibility under S.117 and act as complex case lead for all intensive rehabilitation placements, secure placements and Psychiatric Intensive Care Unit (PICU) placements commissioned by the ICS.
- The post holder will be responsible for negotiation the commissioning of joint packages of care under S.117 with the relevant local authorities, where GMICB is the responsible commissioner. This includes resolving disputes regarding who is the responsible commissioner and negotiating funding splits between multiple organizations both within arbitration and legal settings. This responsibility is supervised and supported by the Deputy Director for CHC
- The post holder will be responsible for presenting Individual applications for individuals with highly complex needs for funding to the Stockport funding panels, providing clear recommendations as to the most appropriate and best value package of care and support and, where joint funding is required, the appropriate sharing of financial responsibility between ICS and Local Authority.
- The post holder will chair Stockport Hospital Discharge and Dynamic Support Database group (HD&DSD), and report to the Quality and Governance Committee regarding the implementation of the Transforming Care Programme in accordance with national and local guidance
- The post holder will, in respect of inpatient care commissioned for people with a learning disability, autism or both, be a point of contact for commissioners, CQC and relevant local safeguarding boards.
