This is a 6 month fixed-term position, with the possibility of a permanent opportunity.
Purpose of Post
To support the Support Planning Manager to undertake care needs assessments, work with clients, supporters, professionals and colleagues to devise person centred support plans and set up and attend 6 weekly and annual service reviews to ensure the service enables clients to achieve their maintenance and personal priority outcomes.
Set up and maintain a ‘Menu of Wellbeing Enriching Activities’ to support managers, supervisors and Group Leaders to create tailored activity plans for their clients as well as an Activity Coordinator Champions Network placing wellbeing at the centre of care provision.
Establish and maintain a programme of group activities and celebration events for all clients including those attending the Day Centre.

Main Duties and Responsibilities:
1. Support Care Team Managers to develop, maintain and review person-centred Support Plans, ensuring that all clients have opportunities to engage in meaningful activities that promote choice, independence and wellbeing.
· Ensure that all activities are appropriately risk assessed. This includes supporting clients to safely take positive risks when this can improve their independence, aspirations, and wellbeing.
· Ensure all strategies reflect company policy, regulatory, contractual and best practice guidance.
2. Set up and undertake administration functions associated with the 6 weekly Service review meetings with the Support Planning Manager and relevant Care Managers/House Supervisors. Review sooner if required.
3. Set up and undertake administration functions associated with the annual Service Review meetings (as above) with clients and their supporters. This will also be attended by the Operations Director.
4. Work with the Registered Manager, Care Team Managers, supervisors, and support staff to review support plan strategies following new behaviours, health deterioration, non-attainment of maintenance/ personal priority outcomes or an incident.
5. Maintain a spreadsheet detailing support plan review dates, and status of paperwork/plans.
6. Support the Support Planning Manager to maintain a ‘Menu of Wellbeing Enriching Activities’ to support managers, supervisors and group leaders to create tailored activity plans for their clients
7. Set up and undertake administration functions associated with the Activity Coordinator Champions Network every 6 weeks, to share ideas and explore new activity options, local facilities and clubs available, upcoming events. Update the Activity Menu and Group Activity Programmes.
8. Establish and maintain a programme of group activities and celebration events for all clients including those attending the Day Centre.
· Research activities with other members of the SLT.
· Work with the Activity Champions Network to set up an annual programme of events and update every 6 weeks at the meeting.
· Encourage the development of small and larger friendship groups to undertake activities together such as ten pin bowling, swimming, visits to parks, cycling, coffee morning etc.
· Host and attend main events and celebration functions
· Attend the Kirklees Activity Coordinators Network
· Active are a member of NAPA - attend meetings and read newsletters etc
9. Ensure all documents are stored in compliance with GDPR regulations and securely disposed of in accordance with established retention schedules.
10. Support with any other admin tasks as required
11. Partake in learning and development opportunities and activities, to continuously develop in your profession.
12. Any other duties commensurate with grade as may arise from time to time.
Person Specification
- At least two years’ experience of working within social/personal care, and one year at least at the level equivalent to senior support worker or above.
- It would also be desirable to have experience supporting people who have Autism, Learning Disabilities and/or behaviours that may challenge.
- Working within a team.
- Must hold a full UK driving licence and have full use of a car, insured for business use.
- Ability to obtain an appropriate DBS check.
- Excellent written communication and literacy skills, with the ability to produce clear, concise, and well-structured documentation that is easy to understand by a range of audiences.
- Excellent organisational and time management skills, with the ability to prioritise tasks, manage competing demands, and ensure the smooth coordination of care services and team operations.
- Ability to communicate effectively and sensitively with different people.
- Ability to use initiative and make appropriate decisions.
- Ability to deal sensitively and appropriately with confidential information.
- Be approachable, patient and understanding to form and maintain effective working relationships whilst working in partnership with client/client families, other professionals and staff.
- Sound knowledge of legislation relating to personal and social care, health and safety, and employment and the ability to follow policies and procedures.
- Sound knowledge of Microsoft Word, Excel, Outlook and PowerPoint.
Pay: £31,262.40 per year
