Key Responsibilities:
Assess patients in their homes, developing, implementing, and evaluating individualised care plans
Manage a caseload with the support of the Clinical Lead
Provide leadership and line management to a team of Registered Nurses and Healthcare Assistants
Work flexibly across teams to meet changing service demands and ensure continuity of care
Our core service operates 8:00am – 6:00pm, 7 days per week , and flexibility is essential to support the needs of the service.
This role requires a professional who demonstrates a positive attitude, compassion, resilience, and vigilance, ensuring safe and effective patient-centred care at all times.

What you will be doing

Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
Managing a caseload with support of the Clinical Lead
Line Management and leadership of a team of Registered Nurses & HCAs.
Our core service hours are 08:00 to 18:00, 7 days per week.
