The Role
As a Care Coordination Consultant, you will coordinate live-in care services, match carers to clients, manage schedules, maintain continuity of care, and support the smooth delivery of care packages.
Combining operational coordination, relationship management, workforce planning, and commercial awareness, you will play a key role in delivering exceptional service whilst supporting business growth and operational efficiency. t.
Key Responsibilities
- Build strong relationships with clients, families, and carers.
- Match carers to clients based on skills, experience, personality, preferences, and care requirements.
- Manage and maintain accurate carer and client schedules.
- Ensure continuity of care through proactive workforce planning.
- Respond promptly and professionally to enquiries.
- Support carers throughout placements and maintain regular communication.
About You
We are looking for someone with proven leadership experience and a strong understanding of live-in care services. You will be an excellent communicator with the ability to motivate others, embrace change, and maintain high professional standards.
Essential skills and experience include:
- Excellent communication and relationship-building skills
- Strong organisational and time-management abilities.
- Ability to manage multiple priorities in a fast-paced environment.
- Strong attention to detail and accuracy.
- Excellent problem-solving and decision-making skills.
- Customer-focused approach with strong IT skills, including CRM systems and Microsoft Office.
Desirable Experience
- Health and social care experience.
- Knowledge of live-in care services.
- Understanding of safeguarding, compliance, GDPR, and CQC requirements.
- Experience coordinating schedules, rotas, or workforce planning.
