Are you an organised, confident, and motivated individual? We are seeking a Service Advisor to join our countywide Mental Health Discharge Team. This vital role supports timely, safe, and well-coordinated hospital discharges for adults with mental health needs. You will be the key point of contact, helping to gather and record information, triage incoming referrals, and ensure that data is accurate and accessible for effective decision-making. As a Service Advisor, your responsibilities will include:

Providing courteous advice and referral services to the public, service users, and partner agencies.

Offering information on available services, benefits, and wellbeing issues.

Triage referrals and determine outcomes, prioritising needs and escalating to senior staff when necessary.

Maintaining accurate and up-to-date records to ensure data quality.

Building strong working relationships with local agencies and maintaining resource databases.

Participating in team meetings and contributing to service development.
To succeed in this role, you should have:

A good general education, including GCSEs (A-C) in English and Maths or equivalent qualifications.

Knowledge of current Adult Social Care and Health policy drivers.

Experience in frontline contact with the public in a social care or health setting (paid or voluntary).

Proficiency in using IT applications and maintaining accurate records.
