Summary
We are seeking a CHAP to join our team in the heart of Middlebrough. Based in our care facility, this role is vital in providing personalized care that aligns with our commitment to excellence and compassion. As a CHAP, you will contribute to creating a supportive environment where residents feel valued and respected.
Responsibilities
- Deliver personalized care to residents, ensuring their comfort and well-being.
- Develop and implement individual care plans tailored to each resident’s needs.
- Supervise and support junior staff members, fostering a team-oriented environment.
- Maintain accurate records of resident progress and care activities.
- Liaise with Multi-Disciplinary Teams, including GP’s and District Nurses, to ensure all aspects of a resident’s care are addressed effectively
- Communicate effectively with residents, families, and healthcare professionals.
- Ensure compliance with health and safety regulations.
- Promote an inclusive environment that respects residents' dignity and independence.
- Support the continuous improvement of care practices through feedback and innovation.
- Qualifications
- Proven experience in senior or residential care settings.
- Strong communication skills and the ability to work collaboratively.
- Supervisory or leadership experience is preferred.
- Valid driving license is advantageous.
- Knowledge of care planning processes.
- Proficiency in using IT systems for documentation.
- Compassionate attitude with a strong sense of ethics.
- Ability to adapt to evolving care strategies.
- Successful applicant must hold a Level 3 in Health and Social Care and Chaps Training or be willing to complete the Chaps Training
