Overview
We are seeking compassionate and dedicated Community Care Workers to provide essential support and care to individuals in their homes and community settings. This role involves assisting clients with daily activities, monitoring health, and ensuring their safety and well-being. Ideal candidates will have experience in senior care, home health, or working with individuals with developmental disabilities or memory care needs. The position offers an opportunity to make a meaningful difference in the lives of those requiring assistance with daily living and healthcare needs.
Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Provide medication administration and monitor vital signs in accordance with care plans
- Support clients with meal preparation, cooking, and nutritional needs
- Utilize patient observation skills to monitor health status and report changes promptly
- Operate equipment such as Hoyer lifts and assist with transfers safely
- Maintain detailed documentation of care provided while adhering to HIPAA regulations
- Perform light cleaning duties to ensure a safe and hygienic environment
- Support clients with dementia, Alzheimer’s, or other memory impairments through specialized care techniques
- Escort to appointments or community activities as needed
- Collaborate with healthcare professionals to develop and implement individualized care plans
- Ensure safety protocols are followed at all times, including CPR and first aid procedures when necessary
Requirements
- Prior experience in home & community care, assisted living, nursing home, or long-term care preferred
- Knowledge of patient monitoring, medication administration
- Compassionate attitude with excellent communication skills for working with residents and team members
- Ability to work independently while following established care plans and protocols
- Valid driver’s license may be required for community outings or transportation duties
