About the Role
Tru Home Care is looking for an organised and compassionate Care Coordinator to join our growing domiciliary care team in Tamworth. You will be responsible for coordinating care packages, managing staff rotas, and ensuring our clients receive safe, high-quality, person-centred care.
Key Responsibilities
- Coordinate and schedule care visits efficiently.
- Ensure all care calls are covered and managed effectively.
- Match carers to clients based on needs and preferences.
- Liaise with clients, families, care staff, and healthcare professionals.
- Support new client assessments and care package implementation.
- Maintain accurate records and care plans.
- Monitor staff training, compliance, and availability.
- Participate in the on-call rota as required.
- Support the Registered Manager in delivering high-quality, CQC-compliant services.
About You
- Previous experience as a Care Coordinator, Scheduler, Senior Carer, or similar role within domiciliary care.
- Good understanding of CQC regulations and person-centred care.
- Excellent communication and organisational skills.
- Ability to work under pressure and manage competing priorities.
- Good IT skills and experience using care management systems.
- Full UK driving licence and access to a vehicle.
