Job Title: Diabetes Specialist Nurse

Job Summary:
- · The post holder will be expected to work as part of the integrated specialist diabetes team delivering diabetes care to adults within primary and secondary care setting on rotational basis.
- · Work as part of the MDT in specialist clinics such as ante-natal, pump, young adults and high risk foot clinics
- · Provide specialist expertise, knowledge, advice and education demonstrating a sound understanding of the management of patients in this area
- · Demonstrate a high level of judgement, discretion and decision making in diabetes management within the acute or community setting, with an aim to reduce hospital attendance/admission and reduce length of stay during in-patient care
- · Deliver education including structured programmes to people with diabetes to enable them to self-manage their condition
- · Develop and deliver educational/training programmes and act as a specialist resource for staff and colleagues to improve standards of diabetes care
- · Develop and implement diabetes policies and standards based on national guidelines, as well as provision of specialist care
- · Monitor and improve standards of care through supervision of practice, clinical audit, evidence based practice, learning and development and the provision of skilled clinical leadership
- · Support the service lead with service improvement development
Main Duties and Responsibilities
- To assess, plan, prioritise, implement and evaluate nursing interventions to meet the patients’ needs which may be in the community or acute setting.
- To actively participate in delivering health care for people with diabetes in Stockport; to ensure high quality and safe standards of care.
- · To advise, educate and support clients and carers on the management of diabetes by providing specialised programmes of care in relation to health and illness and the development of self-management techniques including individual and group sessions
- · Adhere to Nursing & Midwifery Council (NMC) Code of Conduct and associated guidelines.
- · Maintain confidentiality in all aspects of care and employment issues.
- · Be actively involved in clinical supervision of nursing and midwifery
- staff.
- · Act as a role model at all times.
- · Maintain relevant professional registration.
- · Maintain appropriate records adhering to Trust and Nursing & Midwifery Council (NMC) guidelines on record keeping
Communications
- To act in a consultative/collaborative capacity to the multidisciplinary team.
- Specifically provide specialist advice and support to primary care clinicians and those within Stockport Foundation Trust including professional colleagues/medical, nursing, paramedical, social services, private sector and voluntary agencies
- To impart highly complex information which requires developed persuasive and reassurance skills in order to empower patients to self-care
- To deal with distressing/emotional circumstances associated with diabetes eg: newly diagnosed patients, amputations, loss of sight, stillbirths, MIs etc.
- The post holder will communicate sensitive, complex or highly complex information relating to patients by means of written or verbal method within the information governance policy.
- The post holder may be required to attend case conference for certain complex patients and offer specialist opinion.
- The post holder will be responsible for facilitating learning packages for health care professionals, and therefore must be competent with presentation skills and computer software programmes.
Responsibility for Patient Care
- · Assess, plan, prioritise, implement and evaluate nursing interventions to meet patients’ needs.
- · Undertaken comprehensive healthcare needs of patients, changing treatment plan as necessary.
- · Provide psychological support for individuals and carers, identifying and involving the appropriate multi-disciplinary services in order to ensure that the individual needs of the patient are met.
- · Be responsible for assessing and recognising emergencies in their speciality and seeking medical advice or input as needed and be able to interpret information and take appropriate action and lead others to do so.
- · Advise and support patients, to manage their symptoms, giving highly specialist advice as appropriate.
- · Provide specialist care, knowledge and expertise, whilst being continually aware of the importance of empowering colleagues, to ensure dissemination of knowledge and best practice.
- · Attend consultant led outpatient clinics and report on cases (as appropriate to the role).
- · Initiate and manage specialist nurse led clinics (as appropriate to the role).
- · Provide highly specialist advice on the phone, liaising with general practitioners, district nurses and hospital medical teams as appropriate.
- · Take a lead role in the organisation and management of multi-professional patient care.
- · Effectively communicate with clinicians as necessary, ensuring that record keeping is accurate and adheres to Trust policy
- · Create and maintain effective communication with multi-disciplinary team (MDT), patients and carers.
- · Undertake and promote practice sensitive to the needs of patients and families from multi-cultural backgrounds.
- · Contribute to and support the work of colleagues in the multi-disciplinary team.
- · Have a working knowledge of the specific disease according to speciality.
- · Ensure all medication to be prescribed is in accordance with the Trust Medicines Management Policy alongside Trust Non- Medical Prescribing Policy. Ensuring that only medicines identified within the role formulary is prescribed.
- · Plan systems of nursing activity, which satisfy agreed standards at Local, Trust, Regional and National levels.
- · Take a lead role in meetings at which broad professional and service issues are discussed.
- · Demonstrate the mandatory competencies of the Trust, attending all required mandatory updates.
- To take appropriate measurements including blood and urine samples during patient assessments
- To teach patients to self-care and to use equipment to improve their glycaemic control eg: blood glucose meters, insulin pens, insulin pumps, glucose sensors etc.
- To uphold the NMC professional code of conduct at all times, to ensure the safety of clients and staff ensuring that NMC registration is up to date
- Develop mechanisms to ensure that client’s views are represented in service development
Planning and organising
- The post holder will have personal responsibility for the day to day management of their case load. Responsibility for planning and organising workload will rest with the post holder alongside any changes within the service need.
- Contribute to effective planning of the service work load and use of resources.
- The post holder will require organisational skills to prioritise workload alongside urgent patient presentation to the service.
- The post holder will be expected to work across both the community and hospital ward setting.
- To assume an active role in self-development up to degree level and beyond by keeping abreast of trends in nursing and diabetes by attending relevant courses, using library facilities, reading relevant literature, attending local, national and international conferences and meeting with specialist representatives.
- To identify, plan and organise group educational courses for patients with T1 and T2 diabetes that enable the service to run efficiently, reducing waiting lists and offering the patient quality experience.
- To produce in-house literature for patients and carers.
- To participate in educational displays as required.
- To submit statistical returns.
- To maintain accurate visiting and mileage records.
- To submit confidential information/reports as required.
- To attend professional meetings as appropriate.
- To organise and facilitate educational meetings within primary care for multidisciplinary clinicians.
Responsibilities for Physical and / or Financial Resources
- · On-going requirement to exert moderate physical effort, e.g. examining/manoeuvring patients, taking bloods etc.
- To manoeuvre furniture and equipment eg: tables, chairs OHPs etc to provide an environment conducive to learning
- Ensure safe and efficient use of equipment, resources and consumables at all times
- To maintain stock control, safe use of equipment and security of stock
- To ensure that the post holder has an ability to demonstrate the financial efficiencies required within the NHS. To be compliant to the ordering process and ensure that any equipment that is ordered is essential to the role.
- Be familiar with the service budget in collaboration with the Service Lead.

- To contribute to the development and implementation of service policy, protocols and guidelines alongside the service lead.

Responsibilities for Teaching and Training
- · The post holder will be responsible for the design and implementation and evaluation of teaching packages and training for health care professionals
- · The post holder will also be responsible for identification, creation or facilitation of one to one, or group teaching packages for patients and care providers.
- · Patient education is pivotal to the role and therefore the post holder must possess an enhanced level of communication
- · Responsible to deliver and review acute trust mandatory training education for health care professionals liaising with the training department.
- · Provide and develop specialist teaching, which will include practical and theoretical sessions to nursing, medical staff, other health professionals and primary care in own speciality and in collaborating with the multi-disciplinary team
- · To deliver quality Type 1 structured education using QISMET standards to ensure continued accreditation of course.To be involved in ongoing evaluation of course and audit of outcomes.
Responsibilities for data and information resources
- · The post holder is responsible for analysing data and information within service audit.
- · The post holder will be familiar with service data and current KPIs.
- · The post holder should be familiar with the service specification and need to understand and demonstrate the current expected delivery data of care.
- · Data must be entered via service system contemporaneously.
- · The post holder will be offered training within all IT data capture systems across both sites – community and acute setting and will be expected to maintain quality data capture.
- · The post holder has a responsibility to be proficient with the use of word, and with other in house service software systems.
- · The post holder is responsible to maintain accurate data within any of the service systems
- · The post holder should be up to-date with mandatory annual Information Governance training.
Research, Development and Audit
- · Participate in existing research and approved clinical trials.
- · Leads clinical audit.
- · Establish and maintain a learning environment within the service of responsibility.
- · Create an environment in which individuals take ownership for their professional development.
- · Establish and maintain evidence-based practice in the clinical environment; ensuring processes are in place for the identification, collection and evaluation of information to support clinical decision-making.
- · Provide support to staff willing to undertake research and education in individual projects.
- · Demonstrate responsibility for professional development using Personal Development Plan portfolio.
- · To ensure all responsibilities within CQUIN/KPI/AQ are seen as a priority.
Physical Skills and Effort
- The post holder must be a driver and hold UK licence.
- To be able to maintain concentration in the light of frequent interruptions and unpredictability of workload.

Hand Hygiene
To assist the Trust in reducing healthcare acquired infections (HCAI’s) all staff should be familiar with all the Trust’s Infection Prevention policies which are appropriate to their role. You are required to attend mandatory training in Infection Prevention and be compliant with all measures known to be effective in reducing HCAI’s”
Safeguarding
All Stockport Foundation Trust employees are required to act in such a way that at all times safeguards and promotes the health and well-being of children, young people and vulnerable adults. Familiarisation with and adherence to the policies and procedures relating to child protection and safeguarding of children, young people and vulnerable adults are adhered to. Data Protection, Confidentiality and Information Governance
The post holder must abide by all relevant Trust and departmental policies including information governance, confidentiality and data protection and, undertake the annual data security awareness mandatory training. The post holder is reminded that any breach of the Trust’s information governance and security policies and procedures, including data protection legislation, will result in disciplinary action.
Data Protection Legislation – the post holder is required to process all personal data relating to patients and staff, whether in paper, electronic or other media, in accordance with the Data Protection Act 2018 and the General Data Protection Regulations (GDPR), ensuring the security and confidentiality of data at all times.
The post holder must not for their own benefit or gain, or to divulge to any persons, firm or other organisation whatsoever, any confidential information belonging to the Trust or relating to the Trust’s affairs or dealings which may come to their knowledge during employment.
