Reporting to the Director of Student Services, the Healthier Me Coach will coordinate and deliver the College's Healthier Me programme, supporting students and clients with learning disabilities and autism to improve their health, wellbeing and independence.
The postholder will build on existing provision to deliver a programme of health and wellbeing activities, working closely with individuals to identify and achieve personal goals through coaching, workshops, community engagement and partnership working. The role requires a motivated, creative and self-directed individual who can work independently while demonstrating the impact of the programme through effective monitoring and evaluation.
Key Responsibilities
- Coordinate and deliver the College's Healthier Me programme, promoting healthier lifestyles and reducing health inequalities for students and clients.
- Support individuals to identify, set and achieve personalised health and wellbeing goals through 1:1 coaching, regular reviews and action planning.
- Plan, coordinate and deliver a range of health and wellbeing activities, workshops, campaigns and events that reflect the needs and interests of students and clients.
- Build on the College's existing health and wellbeing offer by developing new initiatives and responding creatively to emerging needs.
- Develop and maintain effective partnerships with health services, community organisations, leisure providers and other agencies to increase opportunities and improve access to local services.
- Signpost individuals, families and staff to appropriate health, wellbeing and community services, supporting people to access them where appropriate.
- Manage and monitor an allocated caseload, maintaining accurate records and evaluating progress against agreed outcomes and funding requirements.
- Line manage, support and develop the Healthier Me Champion(s), promoting co-production and meaningful involvement within the programme.
- Work collaboratively with curriculum, residential, therapy and support teams to embed health and wellbeing across all aspects of College life.
- Promote participation, independence and informed health choices through person-centred approaches and positive role modelling.
- Collect, monitor and report participation, progression and impact data, ensuring accurate recording and timely completion of monitoring returns.
- Contribute to self-assessment, quality improvement and the ongoing development and sustainability of the Healthier Me programme.
- Maintain an up-to-date knowledge of health promotion, learning disability practice and local health initiatives.
- Undertake any other duties commensurate with the grade of the post.
Person Specification
- Experience of supporting people with learning disabilities and/or autism.
- Experience of delivering health, wellbeing, education or personal development activities.
- Ability to motivate and coach individuals to achieve agreed outcomes.
- Excellent communication and relationship-building skills.
- Ability to work independently, use initiative and develop creative solutions.
- Experience of partnership working with internal and external stakeholders.
- Good organisational, ICT and record-keeping skills.
- Knowledge of safeguarding, equality and diversity, and safe working practices.
- A relevant qualification in health, wellbeing, coaching, education or a related field is desirable.
- Experience of line managing or supporting volunteers, ambassadors or peer mentors is desirable.
- Full UK driving licence is desirable.

This is a fixed-term post funded through a partnership between Derwen College and Mencap as part of the Healthier Me programme. The programme aims to reduce health inequalities experienced by people with learning disabilities and autism by improving access to health information, promoting healthier lifestyles and increasing opportunities to access health, wellbeing and community services.
The Healthier Me Coach will play a key role in delivering the programme across the College, working directly with students and clients to improve health outcomes while developing partnerships and promoting sustainable health and wellbeing initiatives.
