Job summary
The postholder will work as part of the Integrated Community Nursing Service on the Isle of Wight, delivering high quality nursing care to patients in their own homes as part of a rapidly developing community based model. As a Registered Nurse, they will contribute to safe, compassionate and evidence based care, supporting the Trust's commitment to excellence and continuous improvement.
The role involves working flexibly across three locality teams and collaborating closely with a wide range of partners, including Earl Mountbatten Hospice, Urgent Community Response, Allied Health Professionals, GPs, Practice Nurses, Specialist Nurses, mainland hospitals and voluntary or statutory agencies. The postholder will use strong interpersonal and clinical skills to assess, plan and deliver care, ensuring patients receive the right support in the right place.
For this role you are required to hold a full UK driving licence and have access to vehicle that you can use for work that has business insurance cover, as it involves travel across the island. The postholder must be able to work independently within a busy community environment, demonstrate evidence-based practice, and uphold the Trust's values in all interactions with patients, families and colleagues.

It is essential that you have a valid driving licence to drive in the United Kingdom as the role is community based.
Clinical skills in all types of urinary catheterisation, syringe driver management, wound management and diabetes management are desirable, but not essential.
Support and training will be available to ensure safe a safe transition to working in the community setting.
