Overview
We are seeking an experienced Care Coordinator to join our domiciliary care service in Northwood. This is a key operational role, ensuring the smooth coordination of care services, staff rotas, and high-quality support to our service users.
The successful candidate will work closely with the Registered Manager, care staff, and external professionals to ensure care is delivered safely, consistently, and in line with CQC standards.

Key Responsibilities
Care Coordination & Rota Management
- Coordinate daily and weekly care rotas to maintain safe staffing levels at all times
- Manage planned and unplanned rota changes, including sickness, annual leave, and emergencies
- Allocate carers based on skills, experience, availability schedules, and service user needs
- Ensure continuity of care and work towards zero missed or late calls through effective planning, monitoring, and escalation
Staff Management & Support
- Supporting the recruitment and onboarding of care staff
- Maintaining accurate staff compliance and employment records
- Supporting supervision processes and spot checks
- Assisting with attendance monitoring and performance concerns
Field Care Supervision
- Undertaking field care supervision and spot checks in service users’ homes
- Monitoring care delivery to ensure it is safe, respectful, and person-centred
- Providing constructive feedback and guidance to care staff
- Identifying training or performance issues and escalating to the Registered Manager
On-Call Responsibilities
- Participating in the organisation’s on-call rotaand respond to out-of-hours emergencies and incidents
- Escalating concerns appropriately to senior management
- On-call duties are paid separately in line with company policy
Essential Requirements
- Previous experience as a Care Coordinator in a domiciliary care setting (essential)
- Strong working knowledge of CQC regulations and Fundamental Standards
- Experience of rota management and care scheduling
- Experience supporting recruitment and onboarding
- Experience conducting field care supervision
- Strong organisational, communication, and problem-solving skills
- Ability to manage risk and escalate appropriately
- Competent IT skills (care management systems and Microsoft Office)
- Have a valid Driving License and own a car
Desirable (but not essential)
- NVQ Level 3 or above in Health & Social Care
- Experience using electronic care management systems
Job Type: Full-time
Pay: Up to £25,000.00 per year
