Key responsibilities:
The post holder will be part of the Islington Reablement Service In-house provider services.
Reablement enables individuals to regain or maximise their independence through intensive, short-term support, building on their strengths and enabling people to live healthy independent lives for as long as possible in their own homes, or the place they call home. The Islington Reablement Service is for adults with a range of needs including learning disability, physical and sensory needs, mental health and older people both in community and hospital settings across various locations.
The service is currently delivered 365 days from 8am 10pm. However, the future direction is for the services to become increasingly multi agency and the service may need to change in line with service development requirements.
The post holder will complete initial risk assessments with residents in their home environment, considering environmental risks, risks to the person and others involved in their care and support. They will ensure quality monitoring of care and support being provided and act as a Trusted Assessor to assess and prescribe simple equipment, minor adaptations and assistive technology.
The post holder will deliver personal care and support to people in accordance with their personalised support/Reablement plans. This may include personal care tasks, domestic tasks or exercise programs, developed by therapists to support people to achieve agreed outcomes and add to their rehabilitation or recovery at home. Care and support is given in an enabling way and the focus is on enablement and the promotion of independence. The Senior Enabler will work closely with health and social care professionals, who are involved in the care planning and delivery of Reablement care. They will be responsible for gathering feedback from enablers on a person's progress towards achieving their Reablement goals and joint working with the multi-disciplinary team.
The post holder will exemplify best practice in working with adults in strengths based and person-centred way, connecting people with informal, community-based support wherever possible. You and other team members will be fully committed to working in partnership with residents, carers, community-based organisations and other key professionals involved in enabling people to be as independent as they possibly can.
The post holder will report directly to the Reablement Coordinator.
This is a physically demanding role and the post holder will be working in environments that may be challenging. Willingness and ability to travel across the team geographical areas is essential for this post and flexible working over a 7 day working week will be required to fit in with the needs of the service:
- To ensure that all there is a strengths-based approach demonstrated in the delivery of care and support as detailed in the persons Reablement plan.
- To ensure that the Care Act 2014 is implemented in all aspects of their role.
- To work with people using our services, carers, families and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.
- To promptly feedback any progress or changes in a person's wellbeing, physical health or circumstances to the duty manager or emergency services.
- To keep concise legible records and complete documentation which complies with organisational and professional standards.
- To allocate work to staff (scheduling) during normal working hours, in the evenings and at weekends, including planning, duty tasks, organising the cover for annual leave and other staff absence.
- Responsible for managing the duty telephone line including handling telephone calls effectively, taking and emailing messages or redirecting calls.
- To actively recognise the need to liaise with other services and health professionals to ensure best support is provided to meet the needs of the service users and their families.
- To maintain excellent standards of Infection control at all times adhering to universal precautions.
- To attend and contribute to multi- disciplinary and other meetings as required.
