We are recruiting to fill a fixed term 12-month position in the UCLH HF team. This will involve participating in regular ward rounds led by the HF consultants, as well as nurse led reviews on the other days. We provide telephone advice to patients known to the team, and work closely with the community HF teams in the area.
We have a weekly nurse led clinic for timely post discharge reviews and to see any patients that need review at short notice. We currently partake in an additional weekly HF clinic, along with members of the medical team and a consultant pharmacist. This is under review and may evolve into a further nurse led clinic
Currently we refer most patients on to the local community teams or for rapid titration via BHC and do not have a caseload of patients as such, but this may change as and when the clinics do.
Patients are sometimes discharged to the hospital at home service for IV diuretics and if so, we provide virtual review and advice as needed.

- Consider referrals for the inpatient HF ward round and participate in reviews alongside the medical team
- Nurse led reviews of inpatients
- Nurse led clinics for post discharge review and optimisation of medication
- Co-ordinating with community teams
- Dealing with ad hoc calls from patients
- Reviewing clinic lists to ensure appropriateness of patients and that all relevant tests arranged in good time
- Collecting data for input to NICO
