Sanctuary Personnel is currently recruiting for an experienced Band 6 Community Psychiatric Nurse (CPN) to join an Adults Community Mental Health Team (CMHT) based in East Hull.
This is an excellent opportunity for a skilled RMHN looking to work within a supportive community mental health service, delivering high-quality care to adults with a range of complex mental health needs within the local community.
Key Responsibilities
- Managing a caseload of adults within the community mental health service
- Carrying out mental health assessments, risk assessments, and care planning
- Administering depot medication within community settings
- Providing ongoing monitoring, support, and intervention for service users
- Working collaboratively with multidisciplinary teams, carers, and external agencies
- Maintaining accurate clinical records using System One
- Supporting recovery-focused care and promoting independence for service users
Requirements
- Qualified RMHN with valid NMC registration
- Previous experience working within an Adults CMHT setting
- Experience administering depot medication in the community
- Confident using System One
- Strong assessment, communication, and risk management skills
- Full UK driving licence and access to a vehicle
