Bluestones Medical Agency is currently recruiting skilled and compassionate Band 5 Community Nurses to support Adult Community Services within Midlands Partnership University NHS Foundation Trust. This is an excellent opportunity for nurses seeking flexible agency work while delivering high-quality care to adults within community settings.
- Deliver high-quality nursing care to adult patients within the community.

Support individuals with acute, chronic, and complex healthcare needs.

Carry out clinical assessments, care planning, and implementation of treatment plans.

Administer medication and treatments in accordance with NMC guidelines and local policies.

Work collaboratively with multidisciplinary teams, GPs, social care services, and families.

Promote patient independence, wellbeing, and rehabilitation within home and community environments.

Maintain accurate patient records and documentation.

Ensure safeguarding procedures and patient safety standards are always upheld.
- Registered Nurse (RGN or equivalent) with valid NMC registration.

Minimum 6 months recent NHS or community nursing experience.

Community or district nursing experience is desirable.

Strong communication, clinical assessment, and organisational skills.

Ability to work independently and as part of a multidisciplinary team.

Full UK driving licence and access to a vehicle preferred.
