iCare24 Group is currently recruiting CHC Nurse Assessors to work with NHS Trusts and Integrated Care Boards (ICBs) across the UK. We are seeking experienced nurses who are passionate about supporting patients to access Continuing Healthcare funding and ensuring high-quality assessments in line with the National Framework.
As a CHC Nurse Assessor, you will play a critical role in assessing individuals’ care needs and coordinating appropriate care packages.
Your responsibilities will include:
- Conducting CHC assessments using the Decision Support Tool (DST)
- Working collaboratively with multidisciplinary teams (MDTs)
- Identifying patients’ primary health needs and eligibility for CHC
- Supporting patients and families through the assessment process
- Ensuring all assessments are accurate, timely, and compliant with legal and NHS frameworks
- Maintaining comprehensive documentation of assessments

Requirements:
- Registered nurse with NMC registration
- Experience conducting CHC assessments or working within Continuing Healthcare
- Strong understanding of the National Framework for CHC and associated legislation
- Ability to work independently and make professional, evidence-based decisions
- Enhanced DBS (or willingness to obtain one)
