Job Title: Family Practitioner
Location: Crewe, Cheshire
Rate: £30 per hour (Umbrella)

Hours: 37 hours per week
Sanctuary Personnel are recruiting for an experienced Family Practitioner to join a children and families service across Crewe, Cheshire. This is a hybrid role with a mix of office-based, community, and remote working depending on service needs.
This position plays a vital role in supporting children and families through key transitions, including moving from residential care into foster placements or returning to the family home. The focus is on promoting safe, stable, and positive long-term outcomes for children and young people.
Key Responsibilities
Supporting Children and Families
- Work directly with children and young people to prepare them for transitions, helping them understand and process their experiences
- Support birth families and foster carers to meet children’s emotional, developmental, and practical needs
- Deliver interventions that promote stability, identity, and emotional wellbeing
- Provide parenting support to strengthen attachment, resilience, and family relationships
- Offer consistent emotional and practical support during complex transition periods
Multi-Agency Working
- Collaborate effectively with social workers, foster carers, residential staff, education providers, and health professionals
- Contribute to care planning, reviews, and safeguarding meetings
- Ensure the voice of the child is central to all assessments and decision-making
Assessment and Planning
- Contribute to holistic assessments of need, risk, and family strengths
- Support the development and delivery of structured transition and care plans
- Monitor progress and adapt interventions in line with changing needs
Safeguarding and Risk Management
- Identify and respond to safeguarding concerns in line with local policies and procedures
- Work to reduce placement breakdowns and improve long-term placement stability
- Provide crisis intervention where required
Requirements
Essential
- Experience working with children in care, fostering services, or family support settings
- Strong understanding of attachment theory, trauma-informed practice, and child development
- Good knowledge of safeguarding frameworks and procedures
- Excellent communication and relationship-building skills
- Ability to work effectively within multi-agency environments
- Relevant qualification (e.g., Social Work, Psychology, Family Support or equivalent experience)
- Full UK driving licence and access to a vehicle
- Flexibility to work occasional out-of-hours when required
Desirable
- Experience delivering therapeutic interventions or structured parenting programmes
- Training in restorative practice, mediation, or systemic approaches
- Knowledge of Children in Care processes and care planning frameworks
