We are seeking a compassionate, hardworking, and motivated Healthcare Assistant (Band 3) to join our District Nursing Night Service Team in the South East Essex area.

This community-based role provides care during night hours, operating from 21:00 to 08:00, seven days a week, including weekends and bank holidays, with a variety of shift patterns available.
The successful candidate will play a key role in delivering high-quality, patient-centred care within the community. Responsibilities include triaging calls and referrals, providing responsive unplanned care, and supporting patients in their own homes. The role requires close collaboration with Out of Hours services, multidisciplinary teams, patients, and their families.
Strong communication and IT skills are essential, alongside the ability to work independently and respond effectively to changing patient needs in a fast-paced environment.

Mains roles and responsibility will include:
- Triage of patient calls into service
- Referral triage
- Complex holistic assessments
- Hospital avoidance
- Palliative care
- Catheter care/bowel management
- Working collaboratively with the wider MDT
- Wound management
- Care planning
- Liaise with community nurse Team Leaders for future patient visits and allocation for additional support
- Excellent IT skills
