Job Title: Occupational Therapist – Social Services

Sanctuary Personnel is currently recruiting for an experienced Occupational Therapist to join a progressive Social Services team in Ceredigion. This is an excellent opportunity for a skilled practitioner looking to manage a varied and rewarding caseload while supporting individuals to maximise independence and remain safely within their own homes and communities.
This role is largely remote working, offering flexibility and autonomy. However, due to the nature of urgent and time-sensitive cases, candidates must be based locally to the area and able to attend visits when required. Please note that candidates will be responsible for sourcing their own accommodation if relocating for the position.
You will work independently while collaborating closely with multidisciplinary teams, using advanced occupational therapy knowledge to assess needs, recommend interventions, and deliver person-centred outcomes across a range of complex cases.
Key Responsibilities
- Manage a diverse caseload, completing comprehensive occupational therapy assessments and delivering outcome-focused interventions
- Assess the functional impact of physical and mental health conditions on daily living and independence
- Develop tailored support plans designed to improve wellbeing, safety, and quality of life
- Work collaboratively with service users, carers, families, and partner agencies to achieve person-centred goals
- Provide recommendations for housing adaptations, assistive equipment, and environmental modifications
- Undertake manual handling assessments and implement safe moving and handling plans
- Offer guidance, education, and support to individuals and carers on equipment use and independent living strategies
- Maintain accurate records and ensure all practice complies with HCPC standards, safeguarding procedures, confidentiality, and risk management policies
- Respond appropriately to urgent or time-sensitive cases requiring local attendance
Requirements
- Degree or Diploma in Occupational Therapy
- Current registration with the Health and Care Professions Council (HCPC)
- Previous experience within Social Services, Community OT, or Adult Services settings
- Strong knowledge of manual handling, housing adaptations, moving and handling assessments, and assistive technology
- Ability to work independently, prioritise workload effectively, and manage complex cases autonomously
- Full UK driving licence and access to a vehicle is desirable
