Job Overview
Following the award of new framework work, we are looking to recruit an experienced and organised Care Coordinator to support the day-to-day coordination of our Manchester domiciliary care service.
This is a key operational role responsible for coordinating care visits, supporting care staff, and ensuring care is delivered safely, efficiently, and in line with care plans and local authority requirements.
As part of a growing service, the successful candidate will work closely with supervisors and management and, like all senior operational staff, will participate in the on-call rota and may be required to step in and provide hands-on care when needed to ensure continuity of service.
Key Responsibilities
- Coordinate and manage daily care rotas, ensuring safe and effective coverage
- Respond to sickness, emergencies, and last-minute changes
- Communicate clearly with care staff regarding rotas, changes, and expectations
- Liaise with service users and families regarding visit times and care arrangements
- Work closely with Field Care Supervisors to ensure care is delivered in line with care plans and risk assessments
- Support onboarding of new care packages linked to framework work
- Monitor missed calls, late calls, and rota efficiency
- Maintain accurate and timely records using electronic care management systems
- Escalate concerns appropriately, including staffing risks, safeguarding issues, or service disruption
- Participate in the on-call rota, responding to operational issues
About You
Essential:
- Experience working in domiciliary care
- Experience coordinating care visits or supporting rota management
- Strong organisational and problem-solving skills
- Ability to work calmly under pressure and respond to change
- Confident using IT systems and electronic care management software
- Excellent communication skills with staff, service users, and families
- Willingness to participate in the on-call rota
Desirable:
- Previous experience as a Care Coordinator
- Experience working with local authority frameworks
- Knowledge of systems such as Nourish / CarePlanner
- NVQ Level 4 in Health & Social Care (or willingness to work towards)

Job Type: Full-time
Pay: £27,000.00-£29,000.00 per year
