Social Worker – Hospital Discharge Team
Location: Plymouth
Hours: Monday - Friday
Working Hours: 8:45am – 5:15pm
Pay Rate: £25.38 per hour (PAYE), paid weekly every Friday
Start Date: ASAP
Contract: Temporary
On-site Only: No remote working
We are recruiting a Social Worker to join the Hospital Discharge Team in Plymouth. The role supports patients admitted to hospital, ensuring safe and timely discharge planning through appropriate pathways.
This is a fast-paced, on-site role requiring full-time presence within the hospital and active ward-based working.
Key Responsibilities

Complete timely, person-centred assessments and care planning

Support safe discharge planning, including D2A pathways

Work within a multidisciplinary team (MDT) and partner agencies

Apply safeguarding procedures and the Mental Capacity Act (MCA)

Maintain accurate case recording and documentation

Promote independence, reablement, and patient-centred outcomes
Requirements

Qualified Social Worker with HCPC registration

Strong knowledge of adult social care legislation and safeguarding

Experience in assessments, care planning, and MDT working

Understanding of hospital discharge processes (D2A desirable)

Ability to work at pace in a busy hospital environment

Strong communication, IT, and record-keeping skills
Additional Information

Must be based on-site in Plymouth for all working hours
