Summary
We are seeking a compassionate and dedicated Rehab Worker to join our team in the United Kingdom. Based in the local area, you will play a vital role in supporting clients through tailored care plans and home care services, contributing to our mission of empowering individuals to live independently.
- The client is a pleasant, music loving 17-year-old female with a brain injury, living in Maidstone in her family home.
- The team is led by a Case Manager, with regular training and input from Neuropsychology, Speech and Language and Occupational Therapy.
- Work includes supporting the client to outings, socialising, local visits (i.e. shopping, or out for Lunch), day trips. Supporting with planning and organisation in the home and practicing independent living skills. You will be following therapy programmes set out by the treating therapy team and alerting the team to any concerns.
- You would be joining an experienced team, with training and support available.
Responsibilities
- Support clients with daily activities and rehabilitation exercises.
- Develop and implement personalized care plans.
- Assist transportation needs, including driving when necessary.
- Coordinate with healthcare professionals and family members.
- Maintain accurate records of client progress and therapy activities.
- Promote a safe and supportive environment for clients.
- Communicate effectively with clients and team members to ensure high-quality care.
- Adapt to the changing needs of clients to provide consistent support.
- Qualifications
- Proven experience in care work or rehabilitation settings.
- Valid driving license, business insurance and ability to drive clients when required.
- Excellent communication skills in English.
- Basic IT skills for record-keeping and reporting.
- Compassionate nature with strong interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of care planning and home care procedures is preferred.
- Flexibility to adapt to different client needs.
This is a part time role:
Monday 10am -4pm = 6 hours
Wednesday 4.30pm till 9pm. = 4.5 hours
Thursday 4.30pm till 8pm.= 3.5 hours
There may be occasional extra hours during non-term time by mutual agreement.
