We are looking for a caring, reliable, and enthusiastic Outreach Worker to support a young lady to access and enjoy activities within her local community.
This is a rewarding role where you will help promote independence, confidence, and wellbeing by providing one-to-one support tailored to her interests and goals.
Key Responsibilities
- Support the young lady to access community activities, appointments, and social opportunities.
- Encourage independence while providing appropriate support.
- Assist with planning enjoyable and meaningful outings.
- Provide safe, person-centred support at all times.
- Maintain accurate records and communicate effectively with family and the wider support team.
- Follow care plans and risk assessments.
Essential Requirements
- Full UK driving licence and access to a reliable vehicle (essential).
- Good knowledge and understanding of epilepsy, including recognising seizures and responding appropriately.
- A caring, patient, and positive attitude.
- Excellent communication and interpersonal skills.
- Reliable, flexible, and able to work independently.
- Previous experience supporting people with additional needs or disabilities is desirable.
