Care Coordinator
Peak Home Care is a leading care provider operating throughout Chesterfield and surrounding areas. We provide a range of services to the clients we support, enabling our Service Users to remain living at home and keep their independence. In addition to providing Domiciliary care services we also manage supported living packages.

We are on the look out for a Care Coordinator. This role will require you to manage and be accountable for both our Service Users & Staff within the services.
This position would typically suit someone looking for career progression from a Team Leader/ Senior Role who has previous experience of working with individuals with additional needs. You will have a strong support system that is well managed, we genuinely care that the role is the right fit for you and offers you the best home/ work life balance, in turn we only want the best quality candidates that care about their role and the people they support.
The position will require you to be responsible for:
- Be accountable and work in line with CQC & Local Authority Standards
- Report to your Registered manager who will offer you support
- Producing weekly rotas
- Maintain services to High standards
- Regularly review services and enhance the experience of the clients
- Liaising within a multidisciplinary team
- Managing a team of staff – holding regular staff meetings & providing support
- Weekend working may be needed to cover holidays and sickness within the services.
- On call duties on a rota basis so that it is done equally and fairly
- Recruitment of staff
- Ensure Positive support plans are updated and inline with legislation.
- Ensure paperwork and risk assessments are updated.
- Attend meetings and training days
- Have a positive, can do attitude
